To Use Insurance
In order to use your PPO insurance for a lactation visit, you’ll need to get pre-approved with The Lactation Network. Please click the button below to access the pre-authorization page. Please do not book a visit until you have received pre-authorization (usually within one business day). Once you have received pre-authorization, please follow the steps below to book a visit.
*Amount due to Hatch Midwifery. Other co-payments may apply. Completing pre-authorization will help you understand how much of the visit is covered.
To Book a Home Visit
To arrange a time for a home visit, complete the form below with your preferred times. If you would just like the first available time, please specify that.
Please note that home visits using insurance coverage require a travel fee of $80 per visit.
To Book a Clinic Visit
Please use the calendar below to choose a time that best suits you. Once you have selected the time, you’ll be asked if this is an initial visit. If you are coming in for an initial visit, we will send you an intake form to complete before your visit. If you’ve already been seen and would like a follow-up visit, please contact us directly.
